    What is Blepharoplasty?

     

    (16-minute reading with current approaches)

     

    The supraorbital area is the most apparent area in the facial area, that the aging signs and mood are occurred. This area that the eyelashes, eyebrows, forehead, noses and eyelids are perceived as a whole, is the most common area we use gestures/mimics and that covers the eyes which we avoid the most as a reflex.

     

    Sunlight sensitivity, unspecified/undiagnosed visual impairment, sleep disorders, long-term working on computer, dusty environments, emotional temperament, and many other natural reasons can cause wrinkles, ringing, spots and under-eye bags due to excessive and frequent work of the muscles around the eyes.

     

    The reasons such as genetic susceptibility, allergic reactions, food intolerance, vitamin deficiencies, trauma or accidents, wrong aesthetic procedures can cause supraorbital disorders at early ages, and are other factors that make people tired, exhausted and older.

     

    Due to Gestalt perceptual principles, the lower and upper eyelid ptosis is the most common aesthetic supraorbital problem which causes negatively to the aesthetical image of the person. The solution for these problems which are triggered by age, irregular lifestyle, excess alcohol, smoking, gravity, lying position and insomnia, can be often provided with combined approaches.

     

    Due to holistic approaches, the evaluations should not only be planned separately for all layers but also planned in a whole. Rejuvenation (radiofrequency, laser), tightening (HIFU), regenerative (PRP), volumetric (filling), peeling (chemical peeling), supportive (mesotherapy), spot remover (IPL) or shaping (blepharoplasty) surgical procedures should be performed with sessions or in order of priority for skin problems, such as looseness, spotting or sagging.

     

    In the upper eyelid problems, glabella deep grooves can be accompanied by eyebrow ptosis, apparent wrinkles in goose foot. Neurotoxin (chemo denervation) procedures will be added to the treatment options in the lower eyelid for aesthetic shaping of this area.

     

    Combined lower and upper eyelid problems can be solved/removed simultaneously. Most of procedures are performed under local anesthesia and sessions. For the blepharoplasty, which takes short period for final recovery and shaping, the procedure can be performed in 4 seasons.

     

    Upper Eyelid Aesthetics

     

    Upper eyelid aesthetics is operated under local anesthesia or additionally sedation

    The procedures takes approximately 45 minutes for both eyelids.

     

    The surgery is performed by entering the transcutaneous area of the upper eyelid and the scar hidden in this area is not apparent.

     

    At the surgery/operation, the inner/middle and outer fat clusters which are required, are intervened. As a general rule, it is not recommended to extract fat while muscle hernia and relaxation which cause the skin and fat to become apparent in this part are fixed. (Hamra et al.) It is possible to extract partial fat from very specific fat clusters (hypertrophic) or to transfer fat to other facial areas.  The fats in the inner upper eyelid is localized depth, and is rich in stem cells. Muscles are strengthened, excess skin is removed and the surgery is completed with a thin plaster closure. Permanent stitches can be used to heal eyelid ptosis at the same session in the surgery. Transconjunctival approach is an extra-fat extraction procedure with an inner incision in the eyelid.

     

    The patient can be discharged in a few hours after the surgery, can start the work, travel, take a shower and drive a car after 48 hours by resting and applying ice in following day after the surgery.

     

    Thin stripes that are placed on the stitch line are taken out in 5-6 days. Stitches is not necessary to be taken out if under-skin fusible stitches are used. The patient does not need to be dressed and go for a medical examination during this time. Blepharoplasty can be operated in the same session with rhinoplasty or other facial aesthetics procedures.

     

    The most common procedures which are performed together are neurotoxin (after postoperative edema is removed), filling procedures, and other supplementary procedures such as PRP, mesotherapy, fractional radiofrequency for the skin. Although fantastic energy-based device procedures which cause temporary shrinkage in the skin of the upper eyelid are performed easily and quickly, they provide both temporary and low-profile results. Arabian surgeons in the 10th century were the first surgeons who performed this technique in the upper eyelid by utilizing heat effect. (Cauterization)

     

    Light filling

     

    There is no light filling. The products used under this name are Hyaluronic Acid (HA) in their content and the products which may provide rejuvenation by volume / vitamin / mineral and mesotherapy elements. While the functions provide fillings with the volume they provide, they feed the eye area by vitamins, amino acids and antiaging cocktails in their content. The shelf life of the fillings which are applied for a total of 3 sessions once every 3 weeks is approximately 1 year. These fillings which may have side effects should be performed by experienced physicians and by using appropriate technique for patient health and cosmetic safety.

     

    Lower Eyelid Aesthetics

     

    These are the procedures to give shape to the excess skin, muscle and adipose tissues by entering under the eyelash or transconjunctival, under local anesthesia. In addition, the eyelid can be slung with permanent stitches for eyelid ptosis, which is called tarsoplasty. The use of excess fat tissues (tear through) as a filler in the area between cheek and the eyelid may be given as an example for combined approaches. The recovery period is the same in the upper eyelid.

     

    Eyelid aesthetic procedures and surgeries (Blepharoplasty) are common procedures for the rejuvenation and regeneration of the elderly face, requiring combined, personal, holistic approach and sessions. Signs of aging in the eyes are bruising, grooves, bagging, sagging, static wrinkles, spots and deep grooves. The specific problems of each layer should be evaluated individually.
